Report it at the first possible opportunity to secure yourself from identity theft and prevent someone else from misusing that information.

How to Report Your Lost or Stolen Passport

You can submit Form DS-64 online to report loss or theft of your passport. Clicking this link takes you to a secure portal where you need to navigate to "Report Lost or Stolen", and click "Submit". Enter your personal details along with information on the lost/stolen passport.

After submitting this form online, you get an acknowledgment email confirming your request receipt. After a few days, you receive an email or a letter in the mail if you choose the latter, with a confirmation that your passport has been added to the Consular Lost and Stolen Database.

Replacing a Lost or Stolen Passport

If you're in the US and travelling in 2 weeks or less:

Make an appointment with the Passport Agency to meet with them and have your passport replaced in 8 business days or less. Some restrictions may apply, based on your requirements. Submit Form DS-11 to the agency in person. If you have already reported your lost or stolen passport online, you do not need to fill out the paper form - the only exception is if you personally apply at an agency 48 hours after reporting it online.

If you are in the US and not traveling in 2 weeks:

Submit Form DS-11 in person at your authorized Passport Application Acceptance Facility. If you reported your lost/stolen passport online, there's no need to fill this form out on paper. And the same exception as before applies: not unless you apply at an agency in 48 hours or less after submitting a report online.

If you're outside the US:

Contact a US Embassy nearby to have your lost or stolen passport replaced overseas.

Additional Notes About DS-64

Just so you know, passports that are reported lost or stolen are considered invalid and cannot be used for travel. If a lost or stolen passport has been recovered, you need to submit it to the address found on the back of the DS-64 Form. After submitting it, on your request, it can be canceled and returned back to you. If there's no request from your end, it will be destroyed and rendered useless altogether.

Passports reported lost/stolen cannot be revalidated or used in the passport renewal process.

Can I Cancel a Deceased Relative's Passport?

It is recommended by the Department of State that only a family member or executor of the deceased individual passport holder return it for cancellation purposes. The passport is returned once it is canceled.

You need to submit a valid passport and certified death certificate copy as well as a letter requesting cancellation and permanent invalidation of the passport to this address:

Reporting Non-receipt of Passport after Mailing

The National Passport Information Center or the Online Passport Status System sends you a notification that your newly-issued passport has been delivered to your mailing address. However, in certain cases, you may not have received it even after 10 business days. In such a case you need to get in touch with the National Passport Information Center and let them know of the non-receipt. In this case, the DS-86 Form must be completed.

If non-receipt of your passport isn't reported within 90 days of issuance, you need to reapply for the process along with all applicable fees.

Good to Know

In the US, it is prohibited to have more than one valid passport at a time. A statement is required when applying for a US passport if the previously valid one is not submitted. Your statement will detail why the previous one cannot be presented.
